Patient falls in hospitals continue to plague healthcare professionals and cause harm to patients. In 2017, the Centers for Disease Control and Prevention (CDC) estimated that 3,000,000 patients fall in the hospital annually. The literature identifies many different tools and interventions to prevent patient falls. Hospitals need to consider which assessment tools are best for their institution. In addition, patients need education about fall prevention during their hospitalization.The Dionne Egress Test (Egress Test) is a three-step test created by Michael Dionne, a physical therapist. The test was originally created as a screening tool to help staff assess a patient’s gait and ability to safely transfer following bariatric surgery. Since then, the Egress Test has been used in other settings. For this evidence-based practice quality improvement project, in addition to current use of the Morse Fall Scale, the Egress Test was used by nurses to not only determine a patient’s fall risk status but to also assess the patient’s gait and readiness to transfer. The focus of this DNP project was first to determine nursing staff knowledge on fall risk factors, then provide staff education on the purpose of and how to perform the Egress Test, and finally to have nursing staff utilize the Egress Test as part of their everyday practice. The overall goal of the project was to decrease the number of patient falls at the project site.The project findings revealed the Egress Test assisted nurses in determining if their patient was a high or low fall risk. The project lead reviewed falls data 60 days prior to the implementation of the Egress Test and falls data 60 days post implementation of the Egress Test. The average number of falls for the 2 months prior to the Egress Test education was 2.67 per 1,000 patient days; however, over the previous eight quarters the average fall rate is 3.7 per 1,000 patient days. A higher number of falls occurred in the month of November, just prior to project implementation. In the month of November, after project site staff education, five falls occurred; however, these falls occurred with float pool staff who had not been educated on the use of the Egress Test. Based on this finding, float pool staff education occurred in December and January; three falls occurred December and in January there were two falls. Prior to the education on the use of the Egress Test, only 25% of the patients on this unit were scored as a high fall risk; after education on the Egress Test, the number of patients deemed a high fall risk increased to 50%. Recommendations for this project include continued use of the Morse Fall Scale and the Egress Test to assess patient fall risk, patient gait, and patient readiness to transfer, continued tracking of patient falls, and educating additional nurses on the use of the Egress Test. In addition, as nurses and patient care assistants are hired to this unit, they will be educated on the use of the Egress Test during their onboarding process. This DNP project added to a larger discussion within the facility regarding fall prevention and appropriate tools to use to help nurses critically assess their patients’ readiness to safely transfer while in the hospital.
