Introduction:Urinary incontinence is usually treated with an anticholinergic agent or a beta-3 agonist. In the brain injury population, mirabegron, a beta-3 agonist, has been generally prescribed due to its lack of anticholinergic effects on cognition.Studies have suggested that the beta-3 receptor is an important regulator of physiological functions due to the wide-spread distribution in the human body including the brain, cardiovascular system, colon, bladder, and adipose tissue.The pharmacokinetics of mirabegron is as follows: It is readily absorbed, is metabolized in the liver, peaks about 3 hours after dose, and reaches steady state in 7 days. Half-life elimination is approximately 2 days, and it is excreted 2/3 in urine and 1/3 in feces.Mirabegron acts as a substrate for cytochrome P450 3A4 and 2D6, the 2 major enzymes in the CYP450 system, potentially producing clinically significant drug interactions.In addition, a single nucleotide polymorphism has been described in the beta-3 adrenergic receptor that can affect the efficacy or toxicity of mirabegron.We report a case of cognitive impairment following administration of mirabegron and the return to baseline upon discontinuation of the medication.Case Description:A 28-year old male with a medical history of post-concussive syndrome due to several hockey related concussions 10 years ago with associated transient ataxia, memory loss and headaches was prescribed mirabegron by his urologist for overactive bladder. The patient noted worsening cognitive dysfunction, increased irritability, and mood changes within 4-6 weeks after starting this medication.Mirabegron was discontinued with complete resolution of the patientu2019s symptoms.Discussion:Mirabegron, a selective beta-3 adrenergic receptor (ADRB3) agonist, is widely thought to act on beta-3 adrenoreceptors primarily in the peripheral nervous system.However, beta-3 adrenoreceptors are found in the human brain. In animal models, mirabegron has shown an increase in regional brain metabolism greatest in the frontal cortex, with increases in serotonin and tryptophan levels noted in the cortex, hippocampus, hypothalamus, and striatum.Additionally, genetic polymorphisms of the beta-3 adrenegeric receptor could play a role in the adverse effects of mirabegron.Conclusions:Caution should be exercised in the use of novel medications as patients with brain injuries may be more susceptible for potential central nervous system side effects.Further investigation into the potential effects of beta-3 agonists on mood and cognition is needed.Pharmacogenomic studies on patients having adverse reactions may be helpful in further elucidating the effects of beta-3 receptor polymorphisms.

Now battling the Water Dragon, Jason must take shelter in a cave. Wandering the cave he stumbles upon a magical cape. Witht the cape tightly around his neck, Jason emerges from the cave to battle the Water Dragon once more. Realizing Jason has found the Cape of Legend, the dragon stops his battle with the young knight. Eplains what the boy has found, who he really is and the two unlikely friends agree to help one another reach the castle to defeat Er''K. Once the htey reach the castle the dragon is knocked out by a lightening bott thrown by Er''K leaving Jason alone. Scared and feeling defeated himself, Er''K claims victory until the dragon wakes and reminds Jason that he has the Cape of Legend and to use his imagination and the cape to defeat Er''K. Twirlling the cape to move the clouds away to expose the sun, Jason defeats Er''K and saves the kingdom. The Water Dragon then reveals himself, his true colour and his name, Dargon. Jason saves his mom and finding themselves tightly snuggled back on the cound and ready for bed. Until the next adventure.
