The year 2017 represents not only the 200th anniversary of James Parkinson''s original description of Parkinson disease (PD), but also the 50th anniversary of the development of L-DOPA as a replacement therapeutic, and the 20th anniversary of finding the first major PD-causing gene mutation, in alpha-synuclein (PARK1). This is a personal reflection by the physician who helped define the subspecialty of movement disorders, who wrote the classic reference for patients and families, and who played a pivotal role in many of the major breakthroughs in the field since he began his career studying Parkinson''s in 1962.Dr. Roger C. Duvoisin is Professor Emeritus and Founding Chair of the Department of Neurology at Rutgers Robert Wood Johnson Medical School. As the first fellow of the Parkinson Disease Foundation and Director of the Parkinson Clinic at the Neurological Institute at Columbia under Dr. Melvin Yahr, he participated in the early trials of L-DOPA therapy. Determined to clarify the nature of PD, he worked with Dr. Nathan Stadlan to correlate clinical features with samples from the Parkinson''s Brain Bank and documented Lewy body disease as different from post-encephalitic PD. When studies conducted with Dr. Roswell Eldridge at the National Institutes of Health demonstrated similar concordance between monozygotic and dizygotic twins, Duvoisin initially argued among his colleagues against an inherited basis for PD. When he later began observing multiple cases within the families that he treated, he sought evidence to support a genetic component by directing a family study.1 Documenting Lewy body pathology in a large Italian family with dominantly inherited PD2 he reversed course and led his community of neurologists toward accepting a genetic component to the etiology of PD.In 1989, his Department of Neurology at Rutgers was awarded a $3,500,000 endowment-the largest to date in the history of the school-to develop the William Dow Lovett Center for Neurogenetics, a center that continues his legacy of Parkinson''s research.3, 4 In 1996, as his illustrious career as a Parkinson clinician and researcher was winding down, he took a well-earned victory lap at the National Press Club in Washington, DC. As he proudly pointed to a poster identifying the location of PARK1 on chromosome 4, Duvoisin announced to the world the breakthrough that would forever change the direction of PD research.At a recent webinar about the process of drug development, Dr. Story Landis, Director of the National Institute of Neurological Disorders and Stroke at the National Institutes of Health from 2003 to 2014 was asked what happens when a researcher who has invested years pursuing one course of research is confronted with evidence that veers 180 degrees in another direction. Dr. Landis responded by citing Roger Duvoisin''s courage in publicly reversing his own position and advocating a role for genetics in the etiology of PD.
