Objective : Assess pt perceptions of quality and access to gynecological care through the Veterans Health Administration (VHA) versus outside the VHA. Design : Anonymous cross sectional surveys distributed to VHA patients presenting for gynecological care and collected after visits. Patients : A sample of 356 female Veterans presenting for care the Mission Valley VA Women''s Clinic between Nov 16, 2010 and April 10, 2013. Measures : Patients rated the quality and access of current care and over a 12 month period, both within and outside the VHA system. Analysis : The answers to questions regarding quality and access to care at the VA were summarized. Answers to questions comparing the VA versus outside care was dichotomized (better at the VA versus the same or worse) and was analyzed with respect to age (18-39 versus u003e̲40) and race (White, Black, Asian, Hispanic and other). Results : Gynecological care through the VA was rated as better than outside care by 70% and easier to access by 62%. There was no statistical significant difference reported with respect to age or race.

The central autonomic network includes the insular cortex, anterior cingulate cortex, amygdala, hypothalamus, periaqueductal gray, parabrachial nucleus, nucleus of the solitary tract, ventrolateral reticular formation of the medulla, and medullary raphe. These areas: are reciprocally interconnected; receive converging visceral and somatosensory information; generate stimulus-specific patterns of autonomic, endocrine, and motor responses; and are regulated according to the behavioral state, including the sleep–wake cycle. Several components of the central autonomic networks are affected in neurodegenerative disorders characterized by the presence of intracellular inclusions containing α-synuclein. These include multiple system atrophy (MSA), characterized by accumulation of glial cytoplasmic inclusions, and Lewy body disorders, including Parkinson disease (PD), dementia with Lewy bodies, and the so-called “pure” autonomic failure. In MSA, involvement of the rostral ventrolateral medulla may be primarily responsible for orthostatic hypotension; involvement in the pontine micturition area, sacral preganglionic nucleus, and Onuf nucleus is responsible for neurogenic bladder; and involvement of the pre-Bötzinger complex and medullary raphe may contribute to sleep-related respiratory abnormalities. In contrast, Lewy body disorders are characterized by early involvement of the enteric nervous system and cardiac sympathetic ganglia. The dorsal motor nucleus of the vagus is affected both in MSA and at early stages of PD.

The dramatic recent expansion in genomic information has motivated the development of new approaches to characterize gene expression and function. A critical issue for both basic and clinical endocrinologists is the physiological role of genes involved in regulating endocrine functions. Transgenic technologies allow the translation of genotypic information into specific phenotypes by using gene overexpression or loss of specific gene functions. Murine functional genomics is thus of central importance in modem biomedical endocrine research. Although mice are at present, the preferred mammalian species for genetic manipulations because of the availability of pluripotent embryonic. stem cells and inbred strains and the relatively low breeding and maintenance costs, transgenic rats have also been generated and used to study endocrine physiology. The two basic techniques used in the creation of transgenic animal models are integration of foreign DNA into a fertilized oocyte by random chromosomal insertion and homologous recombination in embryonic stem cells that are then introduced into zygotes. Transgenic mice and rats serve as sophisticted tools to probe protein function, as models of human disease, and as hosts for the testing of gene replacement and other therapies. Embryonic stem cell libraries for mouse gene deletion are being developed, which will make it possible to generate knockout mice rapidly and without the need to analyze gene structure, construct targeting vectors, and screen embryonic stem cell clones. A novel approach to transgenesis for the expression of DNA within adult differentiated neuroendocrine cells in vivo is using viral vectors.
