How Individual Differences in Self- and Other-focused Co-rumination Relate to Internalizing Symptoms and Friendship Quality
Co-rumination involves friends spending a great deal of time encouraging each other to excessively discuss problems, with content being largely negative (Rose, 2002). Co-rumination appears to strengthen the bonds between best friends, while ironically exacerbating internalizing symptoms. Co-rumination is conceptualized as a mutual dyadic process, but little is known about the reciprocity of excessive problem discussion. The balance of college students'' (N = 601) self- and other-focused co-rumination with their best friend was assessed via an online survey. Contrary to expectations, inconsistent and weak evidence was obtained for differentiating self- and other-focused co-rumination, and their balance. Specifically, self- and other-focused co-rumination were highly correlated, similarly correlated with other study variables, and not differentiated in exploratory factor analysis. However, the interaction of self- and other-focused co-rumination in a model including individual characteristics and adjustment yielded differentiated results. Friendship intimacy was associated with self-, but not other-, focused co-rumination. Indicating that balance may matter, anxiety was associated with high self-focused co-rumination in the context of low other-focused co-rumination. Additionally, mean levels of all individual traits (rumination, excessive reassurance seeking, social perspective taking, perfectionism, negative problem orientation) and adjustment variables (anxiety, depression, social anxiety, friendship quality) differed as a function of co-rumination balance, as assessed by a one-item direct measure. The validity and utility of distinguishing self- and other-focused co-rumination is contingent on further exploration with dyadic data and perhaps modified assessment. Rumination and excessive reassurance seeking indicated vulnerability for co-rumination, which appears to be a primarily anxious process.

ASSESSING LONGTERM CHANGE IN THE CULTURE OF INTERPROFESSIONAL COMMUNICATION ON THE SURGICAL WARD SHANG-YEE SANDY YEH, HEATHER SMITH, SEAN BENNETT, JOSHUA GREENBERG, KILEY LACELLE, MAUREEN MCGRATH, PETER GLEN, DAVID CARVER, HUSEIN MOLOO, LARA WILLIAMSIntroduction: In 2014, the general surgery quality improvement team aimed to create a sustainable improvement in communication between nurses and resident physicians on the ward by implementing a change in patient handover and communication methods. Here we summarize the long-term changes in perceived collaboration and communication by nurses and residents following this intervention. Methods: A validated survey assessing perceived communication and collaboration was distributed to general surgery residents and ward nurses before the intervention, 9 months after and 2.5 years after the intervention. The surveys before, 9 months after, and 2.5 years after the intervention were completed by 28, 12, and 20 nurses, respectively, and 21, 15, and 21 residents, respectively. A fisher exact test was used to indicate statistical significance in change of survey results before and after the intervention. Results: Immediately post-intervention, nursesu2019 perceived collaboration with physicians significantly improved (39% pre-intervention, 84% post-intervention, p=0.01). Significantly more nurses felt their input was well received (57% pre-intervention, 92% post-intervention, p=0.01). On the delayed survey, however, only 65% of nurses continued to feel their input well received (p=1.0). The resident group perceived a significant change in how well important issues were communicated between team members (17%pre-intervention, 60% post-intervention, p=0.039). This remained statistically significant two and a half years later (66% delayed post- intervention, p=0.001). Conclusion: Overall, we found significant improvements in communication between nurses and residents 9 months post-intervention. However, these changes were not well preserved over time, particularly as perceived by the nursing staff. This suggests that our current interventions are inadequate to make a sustainable change in communication on the surgical ward. In keeping with the other quality initiatives, implementing a more robust, iterative process of evaluation with root cause analysis may enhance the sustainability of such initiatives in improving the culture of interprofessional communication on the wards.
