A Critical Analysis of Continuous Passive Motion Following Total Knee Arthroplasty
Abstract: BACKGROUND AND PURPOSE: The purpose of this critical analysis was to investigate the existing literature to determine whether continuous passive motion (CPM) is an effective intervention following a total knee arthroplasty (TKA). The variables that were examined included range of motion (ROM), pain, postoperative complications, and length of stay (LOS), and most efficacious protocol. SUBJECTS: Twenty articles were reviewed to determine the effectiveness of CPM as an intervention following a TKA. Inclusion criteria included: the use of CPM, mean subject age 50 years or older, heterogeneous sample, a primary source, and utilization of a control group. In addition, three of the following needed to be met: measures ROM, measures pain, measures postoperative complication, and LOS. METHODS: A research organizer was utilized to determine whether a particular article met the specified, predetermined criteria. Following the selection of articles, descriptive statistics were generated to determine the outcomes of pain, ROM, LOS, and postoperative complications in patients utilizing CPM versus a control group. A database was created using Microsoft Access to organize the information from each article in a user-friendly manner. Tables were created, queries were run, and reports were formed to display the information. Effect size was calculated to measure the magnitude of differences between sample means. RESULTS: Sixteen of the twenty articles reviewed measured knee ROM with six reporting statistically significant results in knee excursion at discharge. Eleven articles measured pain as an impairment level variable. Three articles found statistically significant results indicating that the CPM group needed less pain relief through analgesic use. Seventeen articles measured the incidence of postoperative complications. Four reported statistically significant results, indicating that patients who received CPM had a lower incidence of postoperative complications. A statistically significant shorter LOS in the CPM group was reported in four of the articles reviewed. Effect size was negligible for all variables in all of the articles. DISCUSSION AND CONCLUSION: The results suggest that CPM is an effective intervention immediately following a TKA. However, CPM may not be effective in restoring ROM after discharge. In addition, CPM has negligible effects on pain, postoperative complications, and LOS when compared to a control group. Due to the conflicting results, no efficacious protocol was determined in this research study. Further studies are needed to evaluate the effectiveness of CPM following a TKA to determine the benefits of this intervention.

Panel Processing, which began as a state whistleblower suit and ended as a marquee Sixth Circuit decision about the Employee Retirement Income Security Act of 1974, the statutory behemoth more commonly known as ERISA. By tracing the path of Panel Processing and its odd procedural outcome, the piece highlights a possible gap between federal-question jurisdiction and the complete preemption process, a gap which has the potential to lead to unclear and frustrating jurisdictional outcomes at late stages of litigation. A modest tweak to federal practice-the sua sponte scrutiny of all removal notices premised on preemption doctrines-is proposed as the simplest practical solution to this problem.

In January 2009, in the last week of the Bush Administration, the U.S. Department of Education (ED), upon orders from the departing political appointee staff, published the final report in a long running National Evaluation of Upward Bound (UB). The study was conducted by the contractor, Mathematica Policy Research. After more than a year in review, and over a year after the third and final contract had ended, the report was published over objections from the Policy and Program Studies Services (PPSS) ED career technical staff who were assigned to monitor the final Mathematica contract. The report was also published after a "disapproval to publish" rating in the formal review process from the Office of Postsecondary Education (OPE), out of whose program allocation the evaluation was funded. The Mathematica reports from the Upward Bound (UB) study (Myers et. al. 2004; and Seftor et. al. 2009) have had a large impact on policy development for more than a decade. They have resulted in an OMB "ineffective rating" and were used to justify the zero funding requests for all of the federal pre-college programs, UB, Upward Bound Math/Science (UBMS), Talent Search and GEAR UP in President Bush''s budgets in FY 2005 and FY 2006. As the original and final Contracting Officers Technical Representatives (COTRs) for the study within the US Department of Education, it was the official job of Drs. Goodwin and Callahan to provide Technical Monitoring of the Upward Bound evaluation contracts. In the final of three sequential contracts, after concerns about the study were raised, Dr Goodwin and Dr. Callahan conducted a Quality Assurance Review (ED-PPSS QA review), and found that the impact estimations from the study being reported by the contractor were seriously flawed--so much so that the basic conclusions Mathematica made concerning the efficacy of the Upward Bound program were impacted. While they have spoken out before on this topic, Dr. Goodwin and Dr. Callahan are speaking out again in 2014, because of the on-going and recent citations of the erroneous findings from the study in Congressional testimony, policy briefs, and public speeches (Whitehurst, 2011, Haskins and Rouse 2013; Decker 2013). These erroneous findings continue to do unwarranted and non-transparent serious reputational harm to the Upward Bound program. The ED-PPSS QA review involved an internal review and analysis of all data files from the study, as well as consultation and replication of results by external statistical experts. The data files reviewed included: the initial sampling frame, the baseline survey, five follow-up surveys, student transcripts, 10 years of federal aid files and 10 years of National Student Clearinghouse (NSC) data. The ED-PPSS QA found that the Mathematica reports were seriously flawed, made unwarranted conclusions about the Upward Bound program, and were not transparent in reporting. Moreover statistically significant and educationally meaningful positive impacts on the key legislative goals of the Upward Bound program were clearly found when the study errors were addressed using standards-based statistical methods. These positive impacts are unacknowledged in the Mathematica reports. This report provides: (1) Highlights from the ED-PPSS Review and Re-Analysis: (2) Major Errors Identified in the Technical Monitors'' Quality Assurance Review; and (3) Major Impact Findings from the Re-Analyses.

Research Needs Concerning the Performance of Fiber Reinforced (FR) Composite Retrofit Systems for Buildings and Infrastructure
Throughout the United States, the use of fiber reinforced (FR) composites in building and infrastructure applications has been steadily on the rise. FR composite materials, which include fiber reinforced polymer (FRP) composites and fiber reinforced cementitious matrix (FRCM) composites, are used to repair, strengthen, and seismically retrofit structural components (e.g., columns, beams, walls) of existing buildings and infrastructure. FR composite retrofits have become competitive with more traditional retrofit techniques due to their high strength, light weight, low profile, and corrosion resistance. These FR composite properties have resulted in lower overall costs and more efficient, less labor-intensive installation processes. Furthermore, repair, strengthening, and seismic retrofit of aging structures with FR composites leads to improved building and infrastructure resilience. Since the use of FR composites in the civil engineering field has matured, this report serves to review the state-of-the-art for FR composite performance and identify research needs requisite to advancing the field. The report focuses primarily on externally bonded FRP composites since they are most commonly used in practice in the United States. The research needs were obtained by an extensive literature review and from input received at a national stakeholder workshop at NIST in 2018. The findings of this report describe the materials and structural level research needs prioritized per the subject matter expert opinions received at the workshop. Major materials research needs include improvement of inspector certification processes and improvement or development of test methods that assess the condition of FR composites after installation and long-term use. Other major research needs include identification of bond degradation mechanisms between FRP composites and concrete and relation of accelerated conditioning protocols to long-term exposure outdoors. Structural research needs include development of design standards and guidelines for FR retrofitted components and buildings, improvement of inspection practices, structural-scale experimental studies, and improvement of numerical modeling capabilities. This report is intended to inform future research plans developed by various stakeholders concerning the performance of buildings and infrastructure that are retrofitted or repaired with FR composites.
